The Club agrees to provide the room(s), food, beverages, services and amenities described below and Guest agrees to pay the amounts described below:<\/td>\n<\/tr>\n<tr>\n<td>1.<\/td>\n<td>Date of Agreement<\/td>\n<td>&nbsp;<\/td>\n<\/tr>\n<tr>\n<td>2.<\/td>\n<td>Date of Event<\/td>\n<td>&nbsp;<\/td>\n<\/tr>\n<tr>\n<td>3.<\/td>\n<td>Number in Party<\/td>\n<td>&nbsp;<\/td>\n<\/tr>\n<tr>\n<td>4.<\/td>\n<td>Type of Event<br \/>\n(e.g. banquet, wedding, club meeting, etc.)<\/td>\n<td>&nbsp;<\/td>\n<\/tr>\n<tr>\n<td>5.<\/td>\n<td>Banquet Room Requested<\/td>\n<td>Full Room ($500)<br \/>\nHalf Room ($250)<\/td>\n<\/tr>\n<tr>\n<td>6.<\/td>\n<td>Room Rental<br \/>\n(non-refundable payment required at Date of Agreement)<\/td>\n<td>&nbsp;<\/td>\n<\/tr>\n<tr>\n<td>7.<\/td>\n<td>Food\/Beverage\/Amenities Requested:<\/td>\n<td><textarea cols=\"36\" name=\"ServicesReq\" rows=\"8\"><\/textarea><\/td>\n<\/tr>\n<tr>\n<td>8.<\/td>\n<td>Price for Food\/Beverage\/Amenities<\/td>\n<td>&nbsp;<\/td>\n<\/tr>\n<tr>\n<td>9.<\/td>\n<td colspan=\"2\">Deposit:&nbsp;Payment of 40% of the Price (Deposit) in #8 above is required at the later of Date of Agreement or 120 days before the Date of the Event, with the balance to be paid at or before the Date of the Event.<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 5%;\">10.<\/td>\n<td colspan=\"2\">Cancellation Policy:&nbsp;The Club will refund: 50% of the Deposit in #9 above if cancellation is made in writing and received by the Club 90-119 days before the Date of the Event; zero % of the Deposit in #9 above if cancellation is not received before 90 days before the Date of the Event.
